  At Ofir Orthodontics, we offer patient forms online so that you can complete them prior to your visit. Please click on the DOWNLOAD tab to print your new patient paperwork and bring the completed forms to your office visit. For your protection, please DO NOT email your new patient forms.

  Dr. Ofir is also an active member of several study clubs, including the Invisalign® Elite Study Club and the Downey Study Club. The latter enables him to collaborate and learn from and with highly qualified medical specialists in the handling of difficult cases.
