General Info

Conveniently attached to the Coshocton County Memorial Hospital, our 61-bed, skilled nursing and rehabilitation center provides sub-acute care. Sub-acute care is goal-oriented, comprehensive, inpatient care for the medically complex patient who has had an acute illness, injury, or the exacerbation of a chronic disease. This care is administered immediately after hospitalization. Our sub-acute care setting allows the patient to receive complex care or treatment with a focus on rehabilitation to return to prior functions. Using an interdisciplinary approach, we provide person-centered care encouraging each patient to participate in their care plan. Because of the range of our staffing expertise, sub-acute care is often more intensive than traditional nursing home care. The length of stay can range from 20 to 60 days, depending on the complexity of the patient's condition and the length of time for stabilization or completion of the pre-determined treatment plan. Our priority is for the patient to return home as soon as safely possible.
