- Primary CareWhen you call to to set-up an evaluation, the office staff will complete an intake form with information regarding your child’s personal information, areas of concern, and discipline that is requested. Our office will then request a prescription from your primary care physician that provides the necessary information for insurance companies. Our billing director will check the individual’s insurance plan. It is important to us that we evaluate your child only if and when we have a schedule time slot for therapy, should the evaluation recommend therapy. Our goal is to provide a quality evaluation and consult with the parent regarding the results.
