The Home at Hearthstone is a 96-bed health care facility that provides intermediate care, 24-hour skilled nursing and rehabilitative services. It maintains a dementia and physical medicine rehabilitation units. The center supports various restorative nursing, managed care, insurance, and activity and recreational programs. In addition, it also provides discharge planning services, including home assessment and equipment needs. The Home at Hearthstone provides wound management and peritoneal dialysis care services. The center offers anodyne, intravenous, physical, occupational and speech therapies. Additionally, it provides hospice, psychiatric, case management and respite care services. The Home at Hearthstone is approved by The Joint Commission and is located in Cincinnati.
