- Primary CareYour child’s primary pediatrician will need to complete the form. If your primary care physician is out of the office, we will have to wait until they are back in the office to sign the form. The standard form turnaround is 3 to 4 business days to account for this. If you need a form completed more quickly, we will do so in 1 to 2 business days for $5.00.

- Emergency CareWhen you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing.

- Flu ShotsFlu shots reduce the risk of getting the flu. Some children vaccinated against the flu may still get sick, but their risk of serious complications, such as pneumonia or hospitalization, is greatly reduced.

- ImmunizationsOur pediatricians are able to track and measure your child’s progress using a variety of developmental screenings and examinations, which typically begin only a few days after your baby’s birth. We also provide many newborns with their first immunizations against deadly diseases such as polio, hepatitis B, rotavirus, and more.

- AutismModified Checklist for Autism in Toddlers (MCHAT): We ask that families the MCHAT screening tool to assess risk for Autism Spectrum Disorder (ASD) before the 18 and 24 month check-up appointment.
- Attention Deficit Hyperactivity Disorder (ADHD)Medication Form: Often your child’s school will require a form completed by their pediatrician to have medication given by the school’s personnel. The medications that most often require forms are those prescribed for ADHD, inhalers, EpiPens, and over-the-counter medications. These forms may be obtained from your child’s school.
- Mental HealthCognitive Behavioral Therapy (CBT) is the treatment that works best for several mental health conditions. It involves working on how your thoughts, behaviors, feelings, and how your body feels all go together in a system. We know that they are all connected and that if we change one of those things, all the rest of them change too. With CBT you will learn how you can calm your body down, do a different activity, and handle your upsetting thoughts. It does not mean your upsetting feelings will go away altogether, but just that they will be less loud and distracting. Over time, you’ll pay less attention to them, and more attention to the fun things you are doing or what you are learning in school.
- Behavioral HealthThe behavioral health specialist will submit a separate bill to your insurance company for their service. Depending on your specific coverage, you will either be charged a second copay or be billed for any balance not paid by your insurance (deductible and/or co-insurance). If you have Ohio Medicaid, the services will be covered.
- RadiologyWhen you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers may bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed.
