Since earning his Doctor of Optometry degree from Illinois College of Optometry in 1979, Dr. DeClark's main goal has been to provide the best in family eye care for Chicago. Dr. DeClark is board certified by The American Board of Optometry. The primary mission of the office staff is to facilitate quick and effective treatment of all your ocular needs.
