  Kelly will process your credit card on file (remotely, not on-site) after each session and send you a Superbill which can be submitted for out-of-network physical therapy reimbursement if you plan to seek possible reimbursement. The Superbill has a diagnosis code (ICD10) that reflects the primary issue you’re receiving care for and has therapy (CPT) codes that convey the work we are doing in each session. This document provides your insurance company with the details they require to consider reimbursement.
