- Computed TomographyPlease bring your completed patient forms with you for your visit. These can be mailed to you or found here on our website. Also, please bring a copy of your valid insurance card. In addition, if you have any diagnostic imaging studies, such as x-rays, MRIs or CT scans, these are helpful for your initial appointment.
