  For treatment. We may use medical information about you to provide you with medical treatment or services. We may disclose medical information about you to other doctors and healthcare providers involved in your care. Example: a primary care physician may refer you to a specialist such as a radiologist or a surgeon. The specialist may tell you that you need to be admitted to the hospital for treatment or surgery. All of the doctors in this example will share the medical information about you. This is to coordinate care before, during and after you go into the hospital.

  Please bring your completed patient forms with you for your visit. These can be mailed to you or found here on our website. Also, please bring a copy of your valid insurance card. In addition, if you have any diagnostic imaging studies, such as x-rays, MRIs or CT scans, these are helpful for your initial appointment.

  This practice is limited to the specialty of Orthopaedic Surgery, which is the care of the musculoskeletal system: bones, joints, muscles, ligaments, nerves, and related structures. Within the field of Orthopaedic Surgery, we further subspecialize in the management of spinal disorders. Our physicians have been Board Certified, or are Board Eligible, with the American Academy of Orthopedic Surgery.
