Headquartered in Gastonia, N.C., Footprints Carolina provides case management services through assessment, coordination, monitoring and planning. It offers services to the residents of more than 20 counties in North Carolina who have developmental disabilities, mental illness and related substance abuse issues. The organization offers targeted case management, outpatient treatment and transitional housing services. Footprints Carolina assists its customers and their families in directing and managing their own services through their involvement in planning services and an integrated system of support. It also develops, implements and monitors various crisis plans. The organization maintains the Footprints Advisory Council, which provides an understanding of various issues and a vital customer and community perspective for the senior management teams.
