  1) Fill out and print our Consent Form for Dental Care. Bring the form printed and filled out or emailed for your visit.
• Vaccination
  Proof of current vaccines is required for admission into the boarding facility. If your pet’s vaccines were given at another clinic, you can bring proof from your veterinarian or we can call to have the records faxed to our clinic. If vaccines are not current or no proof is available, one of our staff veterinarians will examine your pet and administer appropriate vaccinations upon admission.
• Behavioral Counseling
  What is the primary reason for this appointment? (please be as detailed as possible about any concerns, including any new lumps/bumps, behavior changes, or changes in mobility)
• Comprehensive Examination
  Pre-Anesthetic Bloodwork -- allows us to diagnose medical conditions that cannot be detected on physical examination before they cause a problem under anesthesia

  What if your pet’s bad breath makes them positively un-kissable? Bad breath may mean there is an issue with your pet’s teeth and gums. But it may also be a sign of a more serious medical condition. This in-depth, medical checkup not only confirms your pet’s current health status, but also helps to prevent future problems, such as obesity, periodontal disease and diabetes. At this time, it also gives us the opportunity to discuss all the ways to keep your pet in good shape and answer all of your questions.
