Hospice Buffalo is a Medicare-certified, hospice program serving more than 5,000 individuals annually. Licensed by the state of New York, the organization offers a wide range of services through interdisciplinary teams of health care professionals, including physicians, nurses, social workers, spiritual caregivers, bereavement counselors, volunteers and other specialists. It develops personalized plan of care for patients and families, and provides 24-hour emergency services. Approved by the Joint Commission on Accreditation of Healthcare Organizations, Hospice Buffalo provides physical, occupational, respiratory and speech therapies, and bereavement and home care services. It offers a wide variety of services, such as care for adult and pediatric patients, cancer, heart disease, stroke, lung, liver and kidney disease, multiple sclerosis, amyotrophic lateral sclerosis, Alzheimer s disease, and AIDS and HIV. Additionally, the organization provides intensive palliative care for the treatment of physical and emotional pain. Hospice Buffalo is located in Cheektowaga, N.Y.
