- ImmunizationsPertussis infection can occur in adults and adolescents even if they have received childhood vaccinations, and infection poses a dangerous threat to vulnerable infants. Although the source of infant pertussis is unknown in many cases, when it can be identified, family members are often the reservoir of transmission.
- AutismAt PHCA, our primary care providers offer consultative visits to address developmental and behavioral health concerns such as developmental delays, autism spectrum disorder (ASD), learning disorders, and other developmental-behavioral health disorders. After a consult, your child may be referred to our dedicated Developmental and Behavioral Health team for an in-house evaluation and follow-up care.
- Attention Deficit Hyperactivity Disorder (ADHD)Form 1 of 2 Parents should print and complete this form prior to your child's appointment for evaluation of ADD/ADHD. Please bring both completed forms to your appointment.
