  INSTRUCTIONS FOR MAKING REQUEST FOR PERMISSION TO REMOVE A SHADE TREE(S) Please fill out the attached form and affidavit and submit to the Bristol Borough Municipal Building. The borough will conduct an inspection of the tree(s) to be removed. Upon said inspection, you will be notified whether or not you can remove the shade tree(s). If permission is granted, a replacement shade tree(s) must be planted within six (6) months of the date of removal of the tree(s). Attached, also, is a List of Suggested Shade Trees along with requirements for planting and general tree care tips.
