Hillside Community Support Services is a program that serves people with chronic health problems and senior citizens in Dodge County, Wis. It provides case management, social work, functional assessment and health education services, as well as bath aide and home-maker visits. Hillside Community Support Services is managed by Beaver Dam Community Hospitals, which is a nonprofit health system that provides a range of health care services. Beaver Dam Community Hospitals employs several physicians who specialize in endocrinology, neurology, gastroenterology, pediatrics, gynecology, psychiatry, cardiology, general and vascular surgeries, orthopedics and urology. In addition to hospital-based services, it provides assisted living and home and hospice care services. Beaver Dam Community Hospitals is accredited by The Joint Commission and is located in Beaver Dam, Wis.
