- Primary CareThe Tuck-in Program is a follow-up program for managing the patient’s safe transition to home, and for ongoing communication with other care providers during the 60-day period following discharge from the facility. This does not replace the caregiver, but provides a link to others who can support a patient’s progress after discharge, like the primary care physician, home care agency care manager and pharmacy.
- Emergency CareWith an average response time of under 5 minutes, IDocsWeb allows residents to receive consultations and evaluations faster than ever before. Residents also enjoy the benefit of treatment where it is safest and most convenient–at bedside. This treatment-in-place practice reduces the risk of cross-infections associated with hospitalizations, and reduces the stress of transportation and unnecessary emergency department visits for care that can be completed safely at our facilities.
- PneumoniaSpeech-language pathologists work with their clients to address swallowing disorders that put individuals at risk for aspiration pneumonia and choking. They also work on improving memory skills, an individual’s ability to talk or establish alternate means of communication or improve their ability to understand speech. Speech-language pathologists play an important role in tracheotomy weaning and recovery.
- ImmunizationsIn addition to her role with Nexion Health, Madhu is involved in a number of organizations, including Sewa International, Aspire, Stop Diabetes Movement, the Louisiana and Texas Dietetic Association and Pratham USA. She frequently participates in immunization drives and health fairs, and she volunteers with local food drives and the Food and Nutrition Conference and Expo. She is a member of the RD council for long-term care facilities.

- Joint ReplacementPhysical therapists work with their clients to improve strength, endurance, ambulation independence and balance. They provide pain management, orthotic and brace training, joint replacement recovery and disease management education. Physical therapists also play an important role in wound healing interventions for chronic vascular ulcers, pressure injuries, non-healing surgical wounds and burns.

- Physical TherapyTara Roberts is a 1995 graduate of Georgia State University with a BS in physical therapy. She joined Nexion Health in 2004 and currently serves as Sr. Vice President of Quality.
- Occupational TherapyMeera Riner holds a bachelor’s degree in occupational therapy and completed her studies at G.S. Medical College in Bombay, India. She also has a master’s in healthcare administration. Meera has served Nexion Health since its inception and currently serves as the Chief Operating Officer.
