General Info

Primary Care: We ask our patients to select one provider to be their ?primary care provider?. This is the provider that takes responsibility for your routine care, i.e. physical examinations and chronic health conditions. In an acute situation (emergency or illness requiring prompt care) your primary care physician may not be available. We request your understanding in these circumstances. Additionally if you are admitted the hospital, or need attention after-hours, you will be called or seen by the on-call physician. You also have the right to change physicians within the practice.
