Operational since 1972, CCHP/SLVHMO (Colorado Choice Health Plans/San Luis Valley HMO) is one of the oldest health plans in Colorado serving its rural Colorado communities. It offers an array of products, such as group health plans, senior plans, third party administration and individual plans. The organization contracts with a comprehensive network of primary care physicians within their communities as well as a specialty and facility network covering the San Luis Valley, Arkansas Valley, Durango, Pueblo, Colorado Springs and Denver.
